I, the participant or his/her representative named above, with NDIS reference number indicated above confirm that I/we have;

  • downloaded a copy of the Participant Induction Pack and;
     

  • read and understand the contents of the Participant Induction Pack;
     

  • been provided with a copy of the Participant Induction Pack

During induction, you'll receive information on how to access services, what to expect from us, and how to get the most out of your NDIS plan. If you have any questions or need additional support, our team is here to assist you. We are committed to making your transition smooth and successful.
